For any of these situations, please contact the CACVT office as your situation will be handled on a case-by-case basis. Please provide as much information as possible such as when you were credentialed in good standing and when it lapsed.

Certification Only (no membership to CACVT)
NOTE: If you do not wish to be a member of CACVT, you may still maintain your certification status in Colorado without joining the association. This is the Certification Only category.

CACVT does oversee this voluntary certification process under the Certification Committee (under the Certification Branch). Please contact the office for details or for a copy of the bylaws.
In order for individuals to maintain CVT status, they must meet defined requirements established by the Certification Committee. Individuals will have no Association benefits but will be considered a CVT in good standing in the State of CO by CACVT standards.

5. and 6. Passed VTNE July-Aug 2010 OR Nov-Dec 2010The following section is only for those who passed the VTNE in July-August 2010 or November-December. Here are the steps to become certified in CO.
1) Please fill out the membership application (note: click here if you do not wish to be a member of the association, but wish to be certified only)

Please fill out completely and either:
A) e-mail or fax to the CACVT office and submit payment via paypal (below) OR
B) print and mail to the CACVT office with check or money order payable to CACVT
2) Submit a COPY of either your diploma or official transcript either by fax, scanned and emailed, or physical mail. We do not need the original, just a copy of the original!
3) Payment of $124 which is good until June 30, 2012. This can be either check (payable to CACVT) or credit card via paypal (after a 3 month grace period, there is a $25 late fee).
Note: if you took the VTNE in another state during this time, and are transferring into CO, CACVT will also require your VTNE scores. Everything else above applies.
Once we receive everything, you will receive a membership kit in the mail in approx. 6-8 weeks. Please contact the office with questions or if you require assistance. Thank you and welcome to the association!

Some schools have a group membership. If you are attending one of these programs, no fees are required. Please just submit the application form. Thank you.

Memberships are for one year only and expire on June 30 of each year. If you are still attending a program, you may reapply for the next year.
12. Application Form for Newsletter ONLY membership
Note: CACVT publications are automatically included in the other membership categories.
This category is ideal for clinics or other groups or individuals who wish to stay current on CACVT activities. It includes a 10-page "Techniques" newsletter on even numbered months, and an E-nformation e-newsletter e-mailed on odd numbered months.
Please fill out the following form, then you will be automatically directed to a payment page.
Costs:
$20/year for Techniques physically mailed to designated address
$10/year for Techniques to be e-mailed to designated e-mail address
$0/year for non-profit groups or special invites (just fill out the form and then exit at the payment page)
