Membership Application Forms

A membership kit will be mailed to you in approximately 6-8 weeks.

If any of this information changes (e.g., new address, name change), it is the responsibility of the applicant to contact the CACVT office. CACVT is not responsible for mail not being delivered or improper recording of CE hours due to a name change not being reported.

Dues may be deductible as professional or business expenses, to the extent allowable by law (please consult a tax accountant). Dues and other contributions to local, state, or national associations are not deductible as charitable contributions for federal income tax purposes.

ATTENTION: People who passed the January 15, 2010 VTNE

Congratulations and welcome to the next step of your career - becoming certified. 

We require 3 specific items in order to become a CVT (keep reading for details):

  1. membership application with appropriate fees
  2. copy of diploma or transcript
  3. VTNE scores

 

1. Membership Application
The "Active Association Membership" is the most popular category for certification. Please fill out the application form and submit with payment (either credit card via Paypal, or check/ money order payable to CACVT). 

Fees (good until June 30, 2010):

For people who passed the January 15, 2010 VTNE: your fee is $30 ($20 if you are a NAVTA member - please specify on your membership application)

For people who passed the June 19, 2009 or the January 16, 2009 VTNE:  your fee is $60 ($50 if you are a NAVTA member - please specify on your membership application). 

For people who passed the June 20, 2008 VTNE:  your fee is $120 ($110 if you are a NAVTA member - please specify on your membership application)

2. Proof of graduation
***  You will also have to provide a COPY of your diploma or transcript to CACVT so we know you officially graduated. This does NOT come directly from the school (unless you have specifically requested your transcripts sent - if unsure, please contact the office), you must submit it yourself. Fax, email scan, or mailed copy are all acceptable.***

3. VTNE Scores
If you tested in CO, we already have your score. If you are moving in from another state, then you must also provide the copy of your passing results from AAVSB.
